From the Sonoma Health Action website: "My Care, My Plan: Speak Up, Sonoma County is a collaborative of organizations and individuals from the private, public, nonprofit and volunteer sectors, including local health care and social service organizations and other community partners, who want to raise awareness about advance care planning. The goal is to help people think about and talk about the kind of care they want—before a crisis occurs—and put these wishes in writing."
